Note: All reports are confidential. Only authorized educators with the proper access code can view these reports, ensuring privacy and compliance with ethical standards.

A questionnaire filled out by the child or adolescent (usually aged 11–17) to assess their own emotional and behavioral well-being.

To be completed by a parent or guardian who has known the child well over the past 6 months. It assesses the child’s emotional and behavioral development in the home and community settings.

This assessment should be completed by a teacher or school staff member who has regularly observed the child in a classroom or school setting for at least 6 months. 

Below 10 years of Age

The assessment relies primarily on observations and insights from parents and teachers rather than individual self-assessment. At this age, children may have limited ability to articulate their emotions and behaviors comprehensively. 

Age 10 years and above

For students ages 11 and above, the assessment incorporates self-assessment as a key component, reflecting the growing ability of older students to articulate their emotions and behaviors.  This self-assessment is complemented by input from parents and teachers.
